Quarter-Century Society - Certificate Request

This form is for use by International Paper Human Resources  or other authorized professionals to order 25-year Quarter-Century Society, Inc. certificates.

Two styles can be ordered :

Style 1 - Blank Certificates (No member name). QCS officer signatures appear at the bottom left of the certificate. A blank line for IP manager signature bottom right. 

Style 2 - Member Name Certificates - You supply the QCS member name. Name will be printed on the certificate. QCS officer signatures appear at the bottom left of the certificate. A blank line for manger signature bottom right.

Do you want any Quarter-Century Society brochures? Brochure is a tri-fold document with basic information about the QCS. New members will receive a personal letter and brochure from the Executive Director during the month of their service anniversary. :
